By becoming a member, you agree to the following:

1. All memberships continue to bill monthly for all active members.

2. All membership benefits are only for active members in good standing and only during the term of their membership.

3. Members in good standing may rollover any unused services to the following months.

4. Members must be 18 or older.

5. If you would like to come more than once a month, you will receive 15%-20% off any additional service or upgrade depending on membership type.

6. If you would like a time extension on a service it will cost an additional $40 per half hour only.

7. Our staff reserves the right to deny service to any client over concerns for any health and/or safety risks, or any signs of inappropriate behavior.

8. Members are required, per their membership agreement, to notify our office of any change in credit card or account information on file prior to the next billing date. A missed payment will lead to termination of the membership.

9. Upon termination of your month-to-month membership, accrued services will expire, and are non-refundable after 60 days from the last billing date.

10. Memberships do not expire. You have the duration of your membership to use your membership services if your account is not frozen, terminated, or canceled.

11. Memberships and their fees are non-refundable.

12. Monthly visit is the service of your choice up to a value of $110: Facial, Acupuncture, Reiki, Massages, and more.

13. Harmony Within Massage Therapy reserves the right to alter these terms without prior notice.

14. Harmony Within Massage Therapy reserves the right to alter membership values at any given time without prior notice, yet will attempt to notify you 30 days prior.

Freezing Membership

• As used below, “active” is defined as a member who has an existing membership agreement.

• There can be times when you may like to freeze your membership. When a membership is frozen, a member cannot use their membership benefits or accrued membership services (massages, facials or acupuncture) for the duration of their freeze period. A member can freeze by contacting our office via phone call. Upon request our front desk staff will send a freeze form via email that must be completed and submitted within 48 hours of being sent. If not completed there will be no change to the membership and the member’s full fees will continue to be billed.

• At least one full membership dues payment has to be made before a membership is eligible to be frozen. Memberships must be in good standing to freeze or unfreeze the account.

• Memberships are allowed to be frozen for up to 3 months. The membership monthly charges will automatically resume after the 3 month period if no notice is given to our office.

• While a member is frozen, they will be drafted $10 per month until they re- activate or discontinue their membership. The total drafted amount is non-refundable.

Canceling Membership

• You must give 30 days notice & pay one more monthly membership fee.

• You will have 60 days to use up any accrued massages.

• There will be no refunds given for services not used.

Cancellation Policy

We have a 24 hour cancellation policy that we enforce. However, if you have illness symptoms please contact us as soon as possible & we will do our best to accommodate a reschedule. If there is an instance of late cancellation within 24 hours or you do not show for the session your membership session will be forfeited. All appointments can be re-adjusted via website or phone a minimum of 24 hours before your scheduled appointment. We ask that you please give us as much notice as possible. Thank you!
